Its done by slitting the sides of the mouth open with a blade about one inch then abusing the victim till they scream and tear open the wounds further.
Inflicting a %26quot;Chelsea Smile%26quot; on yourself would most likely be a horrendous pain especially since if you wanted it to look authentic you would have to tear it open yourself.
This would leave a very visible scar that would require extensive surgery unless it was treated almost immediately so that a cosmetic surgeon could stitch it up with the aesthetic in mind.
For the Joker look i would let it heal naturally irritating the wound with a tooth brush to make the scars raised on the edges and smooth in the middle, for the Kakihara look from ichi the killer i would try to let the wounds heal separately so that they didn%26#039;t fuse together then if you felt like it have the top and bottoms pierced together.
Though this is a very extreme method of body modification, i think if treated properly and with great care you could successfully achieve the results you desire.

xox|||Well, the Joker is a sadomasochist who eenjoys being hurt. But most people don%26#039;t enjoy pain. In the movie, the scars are only so deep, in the comics, the sides of his mouth are cut clear through %26amp; stitched together.
It would hurt A LOT. Esp. with it being wet all the time, it would SO get infected! %26gt;.%26lt; Think is, you can%26#039;t really tell if they were ecternal or internal. If they were external, they%26#039;d get infected from the makeup. Joker%26#039;s not supposed to need the makeup, but the Ledger-Joker does.
You%26#039;d need that crap the dentists give you when they pull your back mollars out. They gave me like 10 injections of that crap %26amp; half my body went numb for an hour %26amp; a half.|||It would be a little painful most probably.
You don%26#039;t NEEED anything to bare it.
You wouldn%26#039;t bleed to death.
Scaring can be repairable for scars but depending on the scar it doesn%26#039;t just make it dissapear!
